Upload the plugin files to the wp-content plugins clicksend-lead-capture-form directory, or install the plugin through the WordPress plugins screen directly..
Activate the plugin through 'Plugins screen in WordPress.
Use 'ClickSend Lead Capture Form menu to edit the settings..
Add the contact form to any page by using the shortcode...
Disappointing at current state the plugin is unable to be used..
