Basic mode the plugin will serve CAPTCHA to all commenters that aren't logged in and don't have only proposed comment.
Repeat commenters will never see CAPTCHA.. Akismet-enhanced mode recommended the plugin will serve CAPTCHA only Akismet defines comment as data less also than the mode.
Upload the folder to the wp-content plugins directory or use WordPress auto-install feature.
