Upload the file to your plugin-directory, default is wp-content plugins.
Activate the plugin through 'Plugins WordPress or Network Activation.
Go to Appearance->Customise and add Site Icon under Site Identity.
After I activated this plugin it broke the site icon, but when I re-applied my site icon all was well.
For some reason the message prompting me to add to home screen worked the couple times, but now it doesn't...
