Activate the plugin through 'Plugins in WordPress.

It basically makes version of the sign-up form that MailerMailer hosts for you but puts it in WordPress..
The one thing to note is that you need to sync WordPress plugin time you make shifts to the sponsored signup form MailerMailer otherwise your updated fields don't display.....
