This plugin is extension for WP Real Media Library plugin that allows you to create folders in system library..
Create thumbnails folders As you can see in the header screenshot not below there is also folder with the thumbnails in it of image.
This feature is not coming with this plugin you need to install WP Real Thumbnail Generator..
Moving already uploaded files This plugin does not allow to move the physically when you move file in Real Media Library because WordPress uses URL's in different places.
Copy the folder into your wp-content plugins folder.
Activate Real Media Library plugin from the plugins admin page.
Upload file to folder and see what happens in your wp-content uploads folder...
