Categories and Tags are tricky sometimes they take bit of experimentation to get right, and when we change the way we want to do things and we end up with mess to clean up.
Upload the folder to the wp-content plugins directory of your WordPress installation.
Activate the plugin through 'Plugins WordPress.

This plugin gives you data about your taxonomies and takes the prudent approach that parent Categories will not be converted.
However, the function of converting tags to categories resulted in deletion of the selected tags without their conversion into categories...
In effect all these categories disappeared, and they don't show up as tags either they are no longer categories or tags.
