Download and extract plugin issues to wp-content plugin directory..
Activate the plugin through WordPress admin interface...
I had problem as @micha79 had to gain SSH access to server and remove whole plugin automatically.
This plugin was installed last and had to be automatically removed FTP access from the folder of WordPress installation then the login worked then as before.....
