WP Simple Forms

WP Simple Forms is hybrid between Custom Contact Forms plugin already on WordPress and Google forms.
The interface is very similar to the interface you see on Google forms, allowing you to create text, choice, check box questions, textareas, and drop down menus.
Choose the question type you would like to add..
Add checkboxes, dropdowns and multiple questions on the fly..
Use shortcode to insert the form into any page...
Upload the folder to the wp-content plugins directory.
Activate the plugin through 'Plugins WordPress.
Place on page to insert the form ...
